What’s the difference between Templates 02 and 11?
Template 02 is a request letter you mail to your state vital records office asking them to issue a Letter of No Record. Template 11 is a self-attestation — a sworn affidavit you sign yourself declaring that no birth record exists. They serve different purposes.
